Active Health Announces New Permanent Location at The Exchange in Farmington — Dr. Jon Lis, MD, Returns to the West Hartford Area; Clinic Set to Open April 1, 2026

Active Health, a preventive-health focused medical and fitness practice, today announced the establishment of a permanent location at The Exchange, 270 Farmington Avenue, Unit 127, Farmington, CT. Construction is near completion, and the practice is on track to open on April 1, 2026.

This new Farmington location, situated on the West Hartford line, represents a major milestone in Active Health’s mission to deliver physician-led, prevention-focused primary care integrated with on-site personal training and health coaching.

Active Health is also pleased to announce that Jon Lis, MD, who has been seeing patients at Active Health’s Waterbury location, is transitioning back to the West Hartford area to lead clinical services at the new Farmington location. Community members & former patients will have the opportunity to enroll with Dr. Lis at the new site.
